Chronic heart failure was listed as the most common discharge diagnosis in French public hospitals over the three years from 1995 to 1997, accounting for 108 000 hospital admissions yearly, or 13.3 per 1000 hospital discharges.11 325 of the 108 000 hospitalizations for chronic heart failure per year resulted in fatalities. Age significantly affected both mortality and hospitalization rates for chronic heart failure, with people over 85 having the highest fatality rate (1413.9 per 100 000) and hospitalization rate (65.3 per 1000 hospital discharges), respectively.
A retrospective analysis was performed on all adult fatalities in France from 1992 to 1996 and all adult chronic heart failure discharges from public hospitals in France from 1995 to 1997. First, both in the general population and subgroups separated by age and sex, deaths from chronic heart failure occurred in France with a startling yearly periodicity and peaked in the winter (December through January). The distribution of total monthly deaths varied by over 35%, peaking in January at 20% above average and falling to 15% below average in August.
Being overweight or obese, active smoking and high blood pressure were the most prevalent cardiovascular risk factor. Volunteer firefighters and career firefighters did not differ significantly from one another. The most frequent risk factor is still obesity, particularly among firefighters who are at a higher cardiovascular risk.
